REQUEST FOR PROPOSALS ASPHALT Notice is hereby given that the Board of Public Works and Safety of the City of Lebanon, Indiana, will receive proposals for Asphalt up to 4:00 p.m. Monday, January 9, 2017 at the Lebanon Clerk-Treasurer's Office, Room 118, Municipal Building, 401 South Meridian Street, Lebanon, Boone County, Indiana. At 7:00 p.m. on Monday, January 9, 2017 the proposals will be opened and read publicly at the Board of Public Works and Safety Meeting, Room 208, Municipal Building, 401 South Meridian Street, Lebanon, Boone County, Indiana. Proposals will be for the following: - 2,500 gallons, more of less, of bituminous material in place, grades MC-I, RC-3, RS-2 meeting State Highway specifications, also all grades of AE liquids. - 500 tons, more or less, hot mix No. 11 and/or hot mix No. 12 surface in place meeting State Highway specifications. Hot mix type B surface course (medium texture) State Highway specifications D303.3B. - 200 tons, more or less, of No. 8 or 9 Hot Asphalt Concrete Binder. - Patching Materials, per ton F.O.B. plant. - Patching Materials, per ton delivered to Lebanon Street Department. - M.W.S. winter mix, per ton F.O.B. - M.W.S. winter mix, per ton, delivered to stockpile at Lebanon Street Department. Bids will be submitted exempt from Federal Tax and Indiana Sales Tax. Bidder can bid on all or any select items listed above. Each bidder shall furnish a certified check in the amount of ten percent (10%) of the bid, payable to the City of Lebanon, or a bidder's bond conditioned that they will promptly enter into a contract with the City of Lebanon or furnish a bond guaranteeing performance hereof. Checks or bonds of unsuccessful bidders will promptly be returned. All bids must have completed and signed a Non-Collusion Affidavit as prescribed by the State Board of Accounts and a Bid Offer Proposal for Sale or Lease of Materials, all of which is supplied in the bid packet. Items to be delivered, both time and place on the above-named materials shall be at the direction of the Lebanon Street Commissioner. Local time prevails on all time issues and the Board of Public Works and Safety reserves the right to waive any irregularities or informalities in any bid and to reject any and all bids.
