SECTION NTB - NOTICE TO BIDDERS 1.0 GENERAL 1.1 Notice is hereby given that separate sealed bids will be received by the City of New Albany until 9:30 A.M. (Prevailing Local Time) on Tuesday, April 19, 2016 at the City of New Albany Office of Redevelopment, Room 325, at the City County Building, 311 Hauss Square, New Albany Indiana 47150, for the following work: FAIRVIEW CEMETERY AND WEST HAVEN CEMETERY IMPROVEMENTS MTR/A # 715-2577 1.2 Lump Sum Bids will be received from General Contractors for all work for the Base Bid and all Alternates as shown on the Drawings and hereinafter specified and as set forth on the Contractors Bid Form. 1.3 Contractor's sealed Bid Packages shall include the following properly executed documents submitted in triplicate: - Indiana State Board of Accounts Form 96 (Parts I & II) - Part III - Base Bid - Part IV - Subcontractor Listing Form - Contractor's Qualification Statement - Contractor's Financial Statement - Bid Guarantee 1.4 Bids will be opened publicly and read aloud at the Board of Public Works, scheduled for 10:00 a.m. Tuesday, April 19, 2016. 1.5 A Pre-Bid Conference will be held at the New Albany City/Co. Building, Third Floor, Small Conference Room, Room 315, 311 Hauss Square, New Albany, Indiana, at 2:00 P.M. (Prevailing Local Time) on Wednesday April 13, 2016. All bidders, including subcontractors, are expected to attend. Prior to the Pre-Bid Conference, bidders should obtain bid documents and visit the project site. 1.6 Copies of the Contract Documents are on file and open to public inspection at the following locations during regular office hours: MICHELL TIMPERMAN RITZ ARCHITECTS 319 BANK STREET NEW ALBANY, IN 47150 TELEPHONE: (812) 945-2324 BUILDERS EXCHANGE 2300 MEADOW DRIVE LOUISVILLE, KY 40218 TELEPHONE: (502) 459-9800 CITY OF NEW ALBANY OFFICE OF REDEVELOPMENT THIRD FLOOR, ROOM 325 CITY - COUNTY BUILDING 311 HAUSS SQUARE NEW ALBANY, IN 47150 1.7 Contractors may purchase complete sets of Drawing and Specification bid packets from Don Meredith Company, 2324A Crittenden Drive, Louisville, KY 40217. Contractors may initially review the packet during regular business hours at the office of the Architect and at the City of New Albany Office of Redevelopment. A. All Contractors considering submitting bids shall verify with the office of the Architect, the total number of addenda issued on a project prior to bid date. Addenda may be viewed at the Architect's office or obtained from Meredith's on-line plan room. Failure to do this could result in rejection of the bid. B. Copies of the Contract Documents may be obtained at the following location during regular business hours: Don Meredith Company 2434A Crittenden Drive Louisville, Kentucky 40217 Phone (502) 636-0155 www.dmcplanroom.com 1.8 The work for which bids are requested includes all labor, materials, equipment and items of materials and services required to provide the proper and complete construction of this project. The work shall be constructed in accordance with this Notice to Bidders, Project Manual, Drawings and Addenda, as prepared for this project. 1.9 Bid and Bid Guarantees shall be placed in sealed envelopes and addressed to the Owner, the City of New Albany, Indiana, and shall list the name and address of the Bidder. 1.10 Should any Bidder withdraw his bid or bids within 60 (Sixty) days after the time of Bid Opening or if issued a Letter of Intent, withdraw his bid or bids after the issuance of such Letter of Intent, or fail to execute a Form of Agreement if presented, the Owner may declare the Bid bonds forfeited to the Owner as liquidated damages. 1.11 The Owner reserves the right to (1) accept any bids by a Letter of Intent within 60 (Sixty) days after the time of Bid Opening and a presentation of a Form of Agreement within 30 (thirty) days from the date of such Letter of Intent (2) at any time reject all bids and (3) waive all informalities, irregularities, or errors in the bidding, provided it is not in violation of the Notice to Bidders or the Statutes of the State of Indiana. 1.12 Bidders not issued a Letter of Intent will have their Bid Guarantees returned within five (5) days after the issuance of the Letter of Intent. 1.13 Bid Guarantee: Each bid shall be accompanied by a certified check, cashier's check, money order or a satisfactory bid bond executed by the bidder and an acceptable surety, payable to the City of New Albany, in an amount equal to 5% of the maximum bid. The attorney-in-fact who executes the Bond on behalf of the Surety shall affix to the Bond a certified and current copy of his Power-of-Attorney. 1.14 Bids shall be executed on Indiana State Board of Accounts Form No. 96 (Revised 2005) with Non-Collusion Affidavit, with all sections completely filled in and sworn to as required by the Statutes of Indiana for all bids. 1.15 All prices quoted, are to be free of all Federal Excise Taxes and Indiana State Sales Tax, and upon request of supplier, tax exemption certificate will be executed by the Owner to the successful bidder to cover all items subject to this tax. 1.16 Contractors receiving awards shall furnish an approved Performance Bond, Labor and Material Payment Bond for 100% of the contract which shall be in full force and effect for 12 months from date of acceptance of the work. Bond shall be furnished at time of written notice to proceed with the work. 1.17 Questions concerning the contract documents or bidding procedures shall be directed to the following: Michell Timperman Ritz Architects 319 Bank Street New Albany, IN 47150 (812) 945-2324 1.18 The Contractor shall submit with his bid a completed Contractor's Qualification Statement, AIA Document A305, including the required Financial Statement. 1.19 The Owner reserves the right to reject each and every bid, and to waive informalities, irregularities, and errors in the bidding to the extent permitted by Law. This includes the right to extend the date and time for receipt of bids. 1.20 The City of New Albany encourages the General Contractor, Subcontractors, Vendors, etc., to make every effort to utilize the residents of Floyd County as suppliers for equipment, materials, and work force whenever possible.
