You are hereby commanded to appear before the Judge of said Court at the Decatur County Circuit Court, 150 Courthouse Square #6, Greensburg, Indiana, (telephone (812) 663-8455), on the 6th day of December, 2016 at 10:00 a.m., to attend an Initial hearing/Termination hearing and to answer the Petition for Termination of your Parental Rights in said Child.
