NOTICE TO BIDDERS The Madison County Drainage Board will receive sealed proposals until 8:45 am (local time) on the 28th day of September, 2016, at the Office of Madison County Drainage Board, 16 E 9th Street, Anderson, Indiana, 46016, said proposals to be opened and read at 9:00 AM (local time) on the 28th day of September, 2016, in the Madison County Commissioners and Drainage Board Conference Room, 16 E 9th Street, Anderson, Indiana 46016, for the following: The Reconstruction & Maintenance of the Stanford Baughan and Tributaries Regulated Drain System Green and Stoney Creek Townships, Madison County, Indiana All bids shall be in separate sealed envelopes and identified by the name and address of the Bidder and labeled BID FOR RECONSTRUCTION & MAINTENANCE OF THE STANFORD BAUGHAN AND TRIBUTARIES REGULATED DRAIN SYSTEM. At the above stated time and place, all bids will be opened and read aloud. Any bid received after the above designated time will be returned unopened. Copies of the plans, specifications and Bid documents are available from the Office of the Madison County Surveyor for the non-refundable sum of ONE HUNDRED TWENTY FIVE Dollars, ($125.00). Checks should be made payable to Madison County Drainage Board. Plans may be viewed at the office of Madison County Surveyor, 16 E 9th Street, Anderson, Indiana 46016, 765-641-9638. Requests for PDF copies of the plans, specifications and bid documents shall be made to the Contract Deputy Surveyor for the Project: Rayl Civil Engineering & Land Surveying, LLC. 2230 Malibu Court, Anderson, Indiana; 765-623-0283, barayl@att.net, for the non-refundable fee of Seventy Five Dollars ( $75.00 ). All Bidders must be registered and on the Bidders List at the office of the Madison County Surveyor. The Proposal shall consist of the following properly and fully executed documents: 1. Bid proposals shall be submitted on General Bid Form 96 as prescribed by the Indiana State Board of Accounts. 2. Each bid proposal shall include a properly executed non-collusion affidavit as required by the statutes of the State of Indiana. 3. Bid Security: Pursuant to Indiana Statute I.C. 36-1-9-6, a Bidder shall file a certified check or bid bond with each bid in an amount equal to five percent (5%) of the total bid proposal, as evidence of good faith that the successful Bidder will execute within fifteen (15) consecutive calendar days from the acceptance of the bid proposal, a contract as set out in the specifications 4. Properly executed Employment Eligibility Verification Form. A pre-bid conference will be held on the 21st day of September at 5149 S 700 West, Pendleton, In. 46064 at the hour of 10:00 a.m. local time. All prospective bidders are urged to attend: Failure of a bidder to enter into a Contract, execute same and furnish acceptable labor and materials payment bond and performance bond as herein required, shall be cause for forfeiture of certified check or bid bond for liquidated damages to the Madison County Drainage Board. Bidder will not be permitted to withdraw his/her/their bid for a period of SIXTY (60) DAYS from the date of bid opening. The Madison County Drainage Board reserves the right to award the bid to the lowest and or best bidder, waive any informalities in bidding or to reject any and all bids.
