SECTION I - ADVERTISEMENT FOR BIDS 1.0 GENERAL 1.1 Bids will be received by The Housing Authority of the City of Charlestown, Indiana, until 2:00. P.M. (Prevailing Local Time) on Thursday September 8, 2016 in the Board Room of The Housing Authority of the City of Charlestown, Indiana, 200 Jennings Street, Charlestown, Indiana 47111, for the following work. PHASE IV CAPITAL FUND PROGRAM IN 36P025501-15 REPLACEMENT OF RESIDENTIAL HVAC EQUIPMENT AT SELECTED BUILDINGS IN MEADOW WOOD AND WOODRIDGE APARTMENTS THE HOUSING AUTHORITY OF THE CITY OF CHARLESTOWN MTR/A #316-2603 1.2 A Pre-Bid Conference will be held at the Board Room of The Housing Authority of the City of Charlestown, Indiana, 200 Jennings Street, Charlestown, Indiana, at 2:00 P.M.(Prevailing Local Time) on Thursday August 25, 2016. All bidders, including subcontractors, are encouraged to attend. Prior to the Pre-Bid Conference, bidders should obtain the bid documents. The Pre-Bid Conference is for the purpose of permitting Bidders and their subcontractors to thoroughly inspect the areas of the proposed work and to request clarification or interpretation of contractor's questions prior to issuance of an Addendum. 1.3 Lump sum bids will be received from contractors for work as shown on the Drawings and hereinafter specified and as set forth on the Contractor's Bid Form. 1.4 Bids will be opened and publically read aloud. Submit complete bids in triplicate, (3) copies. 1.5 Copies of the Plans, Specifications, Addenda and other proposed contract documents are on file and open to public inspection at the following locations during regular office hours: MICHELL TIMPERMAN RITZ ARCHITECTS 319 BANK ST., NEW ALBANY, INDIANA TELEPHONE: (812) 945-2324 OFFICE OF THE EXECUTIVE DIRECTOR THE HOUSING AUTHORITY OF THE CITY OF CHARLESTOWN 200 JENNINGS STREET CHARLESTOWN, INDIANA 47111 TELEPHONE: (812) 256-6311 1.6 Contractors may purchase complete drawing and specification bid packets directly from the Don Meredith Reprographics Company, 2324A Crittenden Drive, Louisville, KY 40219, (502) 636-0155. Contractors may initially review the packet during regular business hours at the office of the Architect. a. All Contractors considering submitting a quote should verify with the office of the Architect, the total number of addenda issued on a project prior to the proposal due date. Addenda may be viewed at the Architect's office or obtain from the printer's on-line plan room. Failure to do this could result in rejection of the Proposal. 1.7 The work for which proposals are requested includes all bonds, insurance, labor, materials, equipment, and items of materials and services required to provide the proper and complete construction of this project. The work shall be constructed in accordance with this Advertisement For Bids, Drawings and Project Manual as prepared for this project by Michell Timperman Ritz Architects. 1.8 Bids and Bid Guarantees shall be placed in sealed envelopes and addressed to: Laura Dabney, Co-Executive Director Leigh Bowyer, Co-Executive Director The Housing Authority of the City of Charlestown, Indiana 200 Jennings Street Charlestown, Indiana 47111 Sealed envelope shall list the name and address of the Bidder. 1.9 Bid shall be executed on Indiana State Board of Accounts Form #96 (current edition) with Non-Collusion Affidavit, sworn to as required by the Statutes of Indiana, for all bids of $5,000.00 or more. All other data required by Bid Form 96, Part II and Part III must be submitted as required by Bid Form No. 96. 1.10 Each bid package shall be accompanied by certified check or bank draft, payable to The Housing Authority, City of Charlestown, Indiana, U.S. Government Bonds, or a satisfactory Bid Bond executed by the bidder of an acceptable surety in an amount equal to five percent (5%) of bids, including all add alternates. 1.11 The successful Bidder shall be required to provide a 100% Performance Bond and Labor and Material Payment Bond before starting any work or placing materials on the job. The Bonds shall be received within ten (10) days of receipt of Notice To Proceed. 1.12 All prices quoted, are to be free of all Indiana State Sales Tax, and upon request of supplier, a tax exemption certificate will be executed by the Owner to the successful bidder to cover all items subject to this tax. 1.13 Attention is called to the provisions for equal employment opportunity, and payment of not less than the minimum salaries and wages as set forth in the specifications must be paid on this project. 1.14 Attention is called to the fact that the Contractor must take affirmative action to insure that applicants are employed, and that employees are treated during employment, without regard to their race, color, religion, sex or national origin. 1.15 Contractors are required to comply with the Provisions of Section 3 as set forth on 24 Part CFR B5 as follows: A. A good faith effort to utilize Section 3 area residents as trainees and employees. B. A good faith effort to award contracts for work in connection with the Project on Section 3 business concerns. 1.16 Attention is called to a request that the Contractor shall make a best efforts goal to achieve a state goal of 10% minority and/or women business enterprise participation in this project. 1.17 All Bidders are required to submit the following with each triplicate copy of their proposals: 1. Contractor's Bid for Public Works, Form 96, as prescribed by the State Board of Accounts of Indiana, complete with Parts I - III. 2. HUD Form 5369A, Representatives Certification and Other Statement of Bidders. 3. Bid Bond, Certified Check or Bank Draft 4. Previous Participation Certificate Form 2530. 5. Contractor's Insurance Certificates with coverages required. 6. Acknowledgement of all addenda issued on the project. 7. Contractor's Financial Statement. (Part III of Form 96) 1.18 Should any Bidder withdraw his bids or bids within forty five (45) days after the time of Bid Opening or if issued a Letter of Intent, withdraw his bid or bids after the issuance of such Letter of Intent, or fail to execute a Form of Agreement if presented, the owners may declare the Bid Bond forfeited to the Owners as liquidated damages. 1.19 Bidders not issued a Letter of Intent will have their bid Guarantees returned within 5 days after the issuance of a Letter or Contract to the successful contractor. 1.20 Bids may be held by The Housing Authority, City of Charlestown, for a period not to exceed forty five (45) days from the date of the opening of the Bids for the purpose of reviewing the Bids and investigating the qualifications of Bidders prior to awarding the Contract. Bidders shall state in HUD form 5369A, Article 6(d) if a longer acceptance period is offered. 1.21 The Housing Authority of the City of Charlestown reserves the right to reject any or all bids or to waive any informalities in the bidding process, to the extent allowable by law.
