The above-named Petitioner, Thelma Theobald, has filed a verified petition for an order to the Indiana Bureau of Motor Vehicles to issue a Replacement Certificate of Title. 2. The allegations and statement of fact set forth in the petition are true and accurate. 3. The Petitioner is the legal owner of an automobile described and identified as follows: Year: 1928 Make: Buick Model: 26 VIN:52-218-507 4. The Petitioner acquired legal title to the automobile on or about 07-23-66. 5. The original Certificate of Title issued to the Petitioner has been destroyed and no duplicate Certificate of Title exists. 6.
