Evans STATE OF INDIANA SS: CARROLL COUNTY IN THE CARROLL COUNTY CIRCUIT COURT CAUSE NO. 08C01-1606-AD-17 IN RE THE MATTER OF THE ADOPTION OF: KATELYN MARIE ROE CASSONDRA KAY EVANS, Petitioner. NOTICE OF ADOPTION Heather M. Roe is notified that a petition for adoption of a child, named Katelyn Marie Roe, born to Heather M. Roe on June 1, 2007 was filed in the office of the clerk of Carroll County Circuit Court. The petition for adoption alleges that the consent to adoption of Heather M. Roe is not required because she has failed to significantly communicate with the child for over one (1) year and has failed to support the child when ordered to do so. If Heather M. Roe seeks to contest the adoption of the child, Heather M. Roe must file a motion to contest the adoption in accordance with IC 31-19-10-1 in the above named court not later than thirty (30) days after the date of service of this notice. If Heather M. Roe does not file a motion to contest the adoption within thirty (30) days after service of this notice the above named court will hear and determine the petition for adoption. The consent to adoption of Heather M. Roe will be irrevocably implied and Heather M. Roe will lose the right to contest either the adoption or the validity of Heather M. Roe's implied consent to the adoption. No oral statement made to Heather M. Roe relieves Heather M. Roe of Heather M. Roe's obligations under this notice. This notice complies with IC 31-19-4.5-3 but does not exhaustively set forth a person a person's legal obligations under the Indiana adoption statutes.
