STATE OF INDIANA COUNTY OF FLOYD FLOYD COUNTY CIRCUIT COURT NEW ALBANY, INDIANA IN THE MATTER OF A CHILD TO BE A CHILD IN NEED OF SERVICES J.H. and A.W. (Minor Children) And Chanda Balmer, Mother, John Hardin, Father And Adrian Wood, Father (Parents) CASE NO.: 22C01-1601-JC-54 22C01-1601-JC-55 SUMMONS FOR SERVICE BY PUBLICATION & NOTICE OF CHILD IN NEED OF SERVICES HEARING TO: Chanda Balmer, Mother Notice is hereby given to the above noted parent, whose whereabouts are unknown, and who is the named parent of J.H. (DOB: 8/11/00) and A.W. (3/30/11), that the Indiana Department of Child Services has previously filed its Verified Petition Alleging the Children to be in Need of Services, in accordance with Indiana Code 31-34-1 et seq., and 31-34-9-3, and that an adjudication hearing was held, and the children were found to be in need of services. A Permanency Hearing is now set at which Chanda Balmer, mother, must appear is scheduled for the 24th day of August, 2016 at 9:00 A.M. at the Floyd County Circuit Court, 311 Hauss Square, #235, New Albany, IN 47150. At said hearing, the court will consider a change of permanency plan and evidence thereon and will render its decision as to whether the permanency plan for the above named minor children should be changed from reunification to change of custody, and shall enter adjudication accordingly. In the Permanency Hearing, the court shall determine, among other things; (1) whether the children's case plan, services, and placement meet the special needs and best interests of the children; (2) whether the Indiana Department of Child Services has made reasonable efforts to provide family services and whether such reunification efforts are still required; and (3) a projected date for reunification if any, and/or the projected date for the children's adoption placement, the children's emancipation, the appointment of a legal guardian for the children, among other permanency arrangements. You must respond by appearing in the case in person or by attorney within ten (10) days after the last publication of this notice, and in the event you fail to do so, the permanency plan may be changed to change of custody without further notice.
