NOTICE TO BIDDERS Notice is hereby given that the sealed bids will be received by the Floyd County Board of Commissioners until 4:00 PM local time on the 21st day of June, 2016 for construction of the following projects: FLOYD COUNTY BRIDGE REHABILITATION PROJECT - JOB NO. 15149 BRIDGE 12 - BANNET ROAD OVER LITTLE INDIAN CREEK, which consists of removing and disposing of the existing steel guardrail on both bridge approaches, installing a new steel guardrail, surface milling ^ inch of the concrete deck and apply 1 ^ inches of latex modified concrete overlay, surface milling the road approaches and installing HMA surface, and installing compacted aggregate on both sides of the new HMA surface. BRIDGE 55 - CHAPEL LANE OVER LEWIS BRANCH OF JACOBS CREEK, which consists of removing and disposing of the existing steel guardrail on both sides of the road and bridge, installing a new steel guardrail, removing and replacing three (3) foot wide bridge concrete deck section, surface milling the bridge approaches and installing HMA surface, and installing compacted aggregate on both sides of the new HMA surface. BRIDGE 103 - ASHBURY DRIVE OVER BALD KNOB CREEK, which consists of removing the existing concrete bridge railing and constructing a new concrete railing, removing and replacing concrete sidewalk and rolled concrete curb, surface milling the existing bridge approaches and installing HMA surface, and installing compacted aggregate on both sides of the new HMA surface. BRIDGE 33 - MALINEE-OTT ROAD OVER GEORGETOWN CREEK, which consists of removing and disposing the entire existing steel guardrail on both sides of the road and bridge, installing a new guardrail system on both sides of the road and bridge, surface milling both bridge approaches and installing HMA surface, and installing compacted aggregate on both sides of the new HMA surface. BRIDGE 40 - SCOTTSVILLE ROAD OVER BIG INDIAN CREEK, which consists of surface milling the entire bridge concrete deck, performing full depth concrete patching on some sections of the bridge deck, placing a latex modified concrete overlay of variable depth, removing and replacing existing cast iron deck drain extension pipes and brackets, installing alternate SS bridge joint, installing two (2) feet thick grouted riprap over geotextile, surface milling both bridge approaches and installing HMA surface, installing compacted aggregated at four (4) corners of the bridge, cleaning all debris from bridge deck and deck drains, and surface sealing the entire bridge concrete from coping line to coping line. BRIDGE 44 - SAINT MARY'S ROAD OVER LITTLE INDIAN CREEK, which consists of repairing damaged portions of the existing prestressed concrete box beams by means of epoxy injection and concrete patching, removing all the existing steel guardrail and installing a new steel guardrail system on both sides of the road and bridge. BRIDGE 45 - PAYNE KOHLER ROAD OVER JACOBS CREEK, which consists of removing and replacing an existing prestressed concrete box beam, removing and replacing a section of the concrete deck, removing all existing steel guardrail and installing a new steel guardrail system on both sides of the road and bridge, surface milling both bridge approaches and installing HMA surface, and installing compacted aggregate on both sides of the new HMA surface. BRIDGE 102 - CHARLESTOWN ROAD JACOBS CREEK, which consists of repairing all spalled or loosed concrete under the bridge concrete arch, replacing existing steel reinforcement where needed and perform all concrete patching as necessary under the bridge, and repairing damaged sections of prestressed concrete box beam by means of concrete epoxy injection. BRIDGE 10 - BALMER-FENWICK ROAD OVER LITTLE INDIAN CREEK, which consists of surface milling both bridge approaches and installing HMA surface, placing a compacted aggregate on both side of the new HMA surface, removing and replacing two and a quarter (2 ^ ) feet wide bridge concrete deck section on both coping lines of the bridge and replacing all damaged or corroded steel reinforcement, removing and resting the existing guardrail, removing and recasting all unsound or loose concrete on the pier nose. At the hour 6:00 PM local time on the 21st day of June, 2016, all bids for this construction will be publicly opened and read aloud at the regularly scheduled meeting of the Floyd County Board of Commissioners. The meeting is held at 2524 Corydon Pike, Suite104, New Albany, IN 47150. The bids shall be unit price for all work shown on the plans and called for in the specifications as detailed in the instructions to bidders in the specifications and submitted in duplicate on the State Board of Accounts Form No. 96. There are NO specified wage rates for this project. A bidder must file with its sealed bid proposal a certified check or bid bond payable to the Floyd County Board of Commissioners in an amount not less than five percent (5%) of the total bid. The above check or bond is to insure the execution of the contract on which such bids are made. The Contractor to whom the work is awarded will be required to furnish, before commencing work, a performance, maintenance and payment bond to extend for a period of one year after final acceptance in an amount equal to the bid price of the contract awarded to said Contractor, and certificates of all insurance required by the specifications. The contract documents, including the plans and specifications, are on file for public inspection at the office of the Floyd County Commissioner's office, 2524 Corydon Pike, Suite 202, New Albany, Indiana 47150. Copies of these documents may be obtained for bidding purposes at the office of Jacobi, Toombs and Lanz, Inc., 1829 East Spring Street, Suite 201, New Albany, Indiana, 47150 by payment of a non-refundable plan fee of One Hundred Dollars ($100.00) for each set of bid documents. Check for the plan fee shall be made out to Jacobi, Tombs and Lanz, Inc. All sealed bids shall be addressed to and turned in to the Floyd County Board of Commissioners, 2524 Corydon Pike, Suite 202, New Albany, Indiana 47150.
