ADVERTISEMENT FOR BIDS: Notice is hereby given that the City of Lebanon, Boone County, Indiana, herein referred to as "Owner" will receive sealed bid proposals for the West Council Drive herein referred to as "Project" until 1:30 p.m. (local time) on June 13, 2016 at the Office of the City Clerk Treasurer, City of Lebanon Municipal Building, One Municipal Plaza, 401 South Meridian Street, Lebanon, Indiana, 46052. Any bids received later than the above time and date will be returned unopened. No conditional bids will be considered. Bids will be publically opened and read aloud at 7:00 p.m. at the Board of Public Works meeting in the Lebanon Municipal Building. DESCRIPTION: The project consists of, but is not necessarily limited to, construction of approximately 2,300 linear feet of a new two lane roadway, with curb and gutter, enclosed storm sewer, railroad spur crossing, and all related civil/site work, instrumentation & control work as specified herein and/or shown on the Road Plans. The project is located between Council Drive and Edwards Drive in the Lebanon Business Park. BID DOCUMENTS: Copies of the Contract Documents may be obtained at Duke Construction Limited Partnership on or after May 23, 2016 for a refundable check of $50.00 per set payable to the "City of Lebanon." Refunds will be made to those bidders that the City of Lebanon deems accepted bid. The contract documents, including plans and specifications, are on file for examination at the Planning Office, City of Lebanon Municipal Building, One Municipal Plaza, 401 South Meridian Street, Lebanon, Indiana 46052. Copies of the Contract Documents, including plans and specifications (digital and hard copy), may be obtained at the offices of Duke Construction Limited Partnership, located at Five Parkwood, 510 East 96th Street, Suite 350, Indianapolis, Indiana, 46240; please contact Julie Holt at 317-808-6409 prior to arrival. Documents may be picked up between the hours of 8:30 a.m. to 11:30 a.m. and between 1:30 p.m. to 4:30 p.m. Bids shall be properly and completely executed on Bid Forms and in the order as identified and contained in the Contract Documents available at the offices as stated above. Each bid shall be accompanied by a completely filled out Form 96 Contractor's Bid for Public Works, including Non-Collusion Affidavit as prescribed by the State Board of Accounts, completely filled out, signed, and notarized as required by the statues of the State of Indiana, Section III of Part II of Form 96 titled, "Contractor's Financial Statement" and acceptable bid security. Any bid not accompanied by the above required items shall be deemed to be non-responsive bid by the Owner. No consideration for escalation on prices can be considered; therefore, Contractors are advised to not include any such escalation clauses in their bids for this project. No bidder may withdraw his/her bid within a period of 90 days following the date set for receiving bids. The City of Lebanon reserves the right to retain the three lowest bid proposals for a period of not more than 90 days, and said bid proposal shall remain in full force and effect during said time. The City of Lebanon City Council further reserves the right to waive informalities and to award the contract to the lowest and most responsible bidder or bidders, all to the advantage of the City of Lebanon, or to reject all bid proposals. BID SECURITY: Each bid shall be accompanied by an acceptable bid security. The bid security shall be a certified check made payable to the Owner or satisfactory bond by an incorporated surety company in good standing and qualified to do business in the State of Indiana in an amount equal to 10% of the bid, said deposit being for the purpose of ensuring the execution of the contract for which the bid is made. BONDS: The Contractor to whom the work is awarded shall be required to furnish a Performance Bond and Payment Bond, each in the amount of one hundred percent (100%) of the contract price, and they must be acceptable to the City of Lebanon and shall remain in effect throughout the entire construction period. In addition, a Maintenance Bond in an amount of 20% of the final contract price for a period of three years from the date of final acceptance of the work will be required.
